The primary concern is for the comfort and well being
of our patient. Prior to beginning any root canal, a complete medical
history is taken. In the appropriate cases, such as medically compromised
patients, a medical consultation is obtained. Risks, benefits, treatment
options, and the prognosis are explained to the patient at this time.
The patient is presented with a consent form to sign, prior to treatment.
Any treatment will then be scheduled with the appropriate Doctor.
At the time of your consultation please be ready
to provide us with the following information:
- Your Doctors referral slip
- Drivers license or valid ID
- A list of all medications you are taking
- Your dental insurance card
- Initial payment/co-pay if applicable
- Please download, fill out and bring in
the forms located in our Patient Forms
section.
NOTE: All patients under age 18 must be accompanied
by a parent or guardian at both the consultation and the treatment
appointment.
Please let our Doctor know prior to surgery if you
have or had any medical conditions that could be of concern, such as,
rheumatic fever, high blood pressure, diabetes, artificial heart valves
and joints etc, or if you are using such medications as, anticoagulant
therapy, heart medication, aspirin, etc.
